List plan revisions

Participants whose plan end date was revised, scoped to the plan manager. Requires X-Plan-Manager-Id header. Ordering: by created_on descending (most recently recorded revision first). Webhook-sourced and plan-info-sourced revisions are merged and then sorted together by date.
